Step 1: The consumer
employer or representative should read and sign the ChoiceCare
Authorization Form and Employer
Appointment of Agent (IRS 2678) Form and return the originals
to ChoiceCare. You will only have to do this once. If you are already
using Acumen or another Fiscal Agent, you will need
to complete the change of agent form and send it to us before we can
take over.
Step 2: Before each
Employee is hired, they must complete the W-4,
I-9, and G-4
Forms. You as the Employer or your Representative must complete a
section of the I-9 form, and must obtain a copy of the employee's
driver's license and social security card. These forms and documents
can be faxed or mailed to ChoiceCare, LLC.
Step 3: Lastly, before
an employee is hired they must be given a copy of the Employee's
Rights & Responsibilities document, which is required
by law before a Background Check is performed, and the employee must
sign and return both the PeopleCheck
Release & Authorization Form and the Fair
Credit Reporting Act Disclosure
Once all of these completed forms are
received by ChoiceCare, you and your employee(s) will be official.
Remember to keep a copy of all forms for your records.
Once your employee(s) are hired
There are some other forms that you will
need, the most important being the employee timesheet. The timesheet
is to be completed every two weeks, be signed by the employee, and
also signed by you or your representative. If the employee wants their
paycheck directly deposited into their checking/savings account, they
must complete and return the Direct
Deposit Authorization Form, and if either you or your
Employee's name or address ever changes, the Change
of Information Form must be completed and returned
immediately. If you terminate an employee, you must provide that person
with a Separation
Notice, according to Georgia Law.
